How to Obtain a Police Report or Records Release
Please download and complete a copy the Request for Records form (PDF). Completed request forms may be submitted to the Department at 403 Highland Avenue, Piedmont, CA 94611. You will need to show identification when picking up your report.
Note: Per Government Code §6254, not all police records are available for release.
Certain contents of a report may not be deemed public information and your copy will be redacted to comply with provisions of the law.
If you are directly involved in a police report, such as being the victim of a crime, or a driver/property owner in an accident, you may request the report by submitting a Request for Release of Information form. Once your request has been received, the Records Division will contact you within 10 days to advise you on the status of your request (Government Code §6254). Please be prepared to present a valid I.D. upon accepting the report. Items with a red asterisk are required before submitting the request.
